Observation chart 是臨床上非常重要的生命徵象紀錄工具，用來持續監測病人的基本生理狀況，除了單純記錄數值之外，Observation chart 也具有識別惡化（deterioration）功能。根據異常數值設定警示範圍，提醒醫護人員何時需要進一步升級通報（escalate care）和處置。

先看一下Observation chart什麼樣，下面會分區塊講解。

記錄方式

1. 呼吸、血氧紀錄方式

注：Nasal Prongs 就是台灣常說的Nasal Cannula

2. 血壓紀錄方式

注：上面箭頭為SBP，下面箭頭為DBP。SBP 落在黃色或紅色區塊，需近一步處置。

3. 心跳紀錄方式

常見 rhythm 縮寫：

• SR = Sinus Rhythm（正常竇性心律）

• ST = Sinus Tachycardia（竇性心搏過速，心跳快）

• SB = Sinus Bradycardia（竇性心搏過緩，心跳慢）

• AF = Atrial Fibrillation（心房顫動）

• AFL = Atrial Flutter（心房撲動）

• SVT = Supraventricular Tachycardia（上心室性心搏過速）

• VT = Ventricular Tachycardia（心室心搏過速）

• VF = Ventricular Fibrillation（心室顫動）

• PACs = Premature Atrial Contractions（心房早期收縮）

• PVCs = Premature Ventricular Contractions（心室早期收縮）

3. 體溫紀錄方式

4. 意識評估（非GCS）

• A = Alert
  病人清醒、能自然回應、知道周遭環境。

• C = New Confusion/Change in behaviour
  新發現的混亂、行為改變。

• V = Voice
  病人對聲音有反應，例如叫名字會張眼或回答。（執行GCS評估）

• P = Pain
  病人只有在疼痛刺激下才有反應。（執行GCS評估）

• U = Unresponsive
  病人完全沒有反應。

5. 疼痛評估

病人若在休息填入R (rest) ; 若在移動、翻身、走動填入M (movement)。

識別和處置惡化 Recognise and responding to deterioration

上面的紀錄方式你可能發現了有黃色和紅色區域，如果有數據落在這個區域裡就需近一步通報和處置。

下面圖表內容不逐一翻譯，建議都要看過，培養閱讀英文內容的習慣！

1. 黃色區域（Yellow Zone）

• 適時調症照護，例如：呼吸急促(SOB)→搖高床頭 ; 頭暈、血壓低→躺平

• 30 分鐘 recheck 一次

• 通報 Nurse In Charge (Team Leader)

• 除了紀錄落在黃色區域，其他臨床表徵也需注意，參考圖表下方 Additional Criteria

2. 紅色區域 （Red Zone）

• Call Code blue/ MER(Medical Emergency Response)。（由醫院急救團隊接手）

• 急救團隊來之前，執行ABCDE assessment

• 除了數據，Additional Criteria 一但發現也是馬上call for help

• 如何Call for help→按下Emergency button, 大聲喊I need urgent help here

• 急救團隊抵達前，不可以離開病人

這個章節提到的 Handbook，是臨床工作中幾乎每天都會使用到的重要工具。

無論是在準備給藥、核對藥物資訊，或確認給藥方式時，都需要依據標準參考資料來確保病人安全，而不是單靠記憶。

技術考時，你不需要記住所有藥物如何準備、要怎麼給，只要知道怎麼查詢正確資訊。

靜脈給藥方式

靜脈藥物給藥方式分為兩種：

Injection: (aka IV push) 直接將藥物透過針筒直接注射到靜脈，Injection 通常要在5分鐘內注射完成。

Infusion: 將藥物稀釋加入點滴(100mls, 250mls, 500mls)裡滴注。

醫生的Order只會寫IV，不會告訴你是哪一種，那要如何知道這個藥物是要Injection 還是 Infusion呢，往下繼續看～

藥物查詢手冊

需上 Australian Injectable Drug Handbook (AIDH) 查詢，這個專門查 injectable medication：

• 稀釋方式 (preparation)

• 相容性（compatibility）

• 注射速度

Australian Medicines Handbook (AMH) 主要查詢：

• 適應症

• 劑量

• 禁忌症

• 副作用

備註：這兩個網站都需要有帳號才能登入，有些醫院還沒電子化會有紙本手冊。

例子

以Vancomycin為例：

1. 先上AMH查詢藥物作用和劑量

2. 再上AIDH查怎麼給和如何準備

由上列資訊可知，這個藥物只能IV infusion，1 g vancomycin 需要給 > 1小時

稀釋方式 : 1 g 使用 20 ml water of injection，再加到200ml fluid

可以相容以上 fluid

提醒：Antibiotic 是需要兩人核對的，記得雙人都要看過藥物怎麼準備

IV fluid chart

病人如需要IVT (Intravenous Therapy) 的，藥囑會在IV fluid order，和藥物order不會在同一個位置 （各家機構的可能有些微差異）

澳洲電子醫療紀錄Electronic Medical Record（EMR）還沒很普及，有些地方是全紙本病例，有的是一半一半，紙本的給藥紀錄單(Medication chart) 在很多公私立醫院、養老院都還在使用，所以還是需要熟悉紙本作業流程。

先來看一下給藥紀錄單長什麼樣，下面會分區塊講解。

給藥核對 Right of medication administration

藥物給藥通常遵循 「六對原則」（Six Rights of Medication Administration）

這六項原則包括：

1. Right Person（正確的病人）

2. Right Medication（正確的藥物）

3. Right Dose（正確的劑量）

4. Right Time（正確的時間）

5. Right Route（正確的途徑）

6. Right Documentation（正確的紀錄）

備註：六對是最核心基本的原則，但根據不同機構可能會更多到八對或十對等…例如：

• Right education（正確的衛教）

• Right Effect（正確的治療效果）

1. Right Patient

   1. 給藥前需確認病人3項身份，核對姓名、生日（DOB）、病例號（UR number)

   2. 使用手圈(wrist band) 及medication chart 雙重核對，不可使用床頭卡核對

   3. 如果病人情況允許，請他說出自己的全名和生日

   4. 詢問病人有沒有過敏史（Allergies），每次給藥都要問一次！

      

      圖片備注：左側紅色區塊是紀錄病人有無藥物過敏史，每次給藥前都要確認，就算上面沒有任何紀錄也要跟病人再次確認！

      例句：
      May I have your full name and date of birth?

      Do you have any allergies?

2. Right Medication

   1. 確認藥物標籤與醫囑是否一致

   2. 確認包裝完整、保存期限

      

      備註：所有藥囑都一定要確認Prescriber signature 醫生有簽名！沒有簽名的藥囑絕對不能給！

      

3. Right Dose

   1. 確認藥物的劑量與濃度是否正確

   2. 上Australian Medicines Handbook網站確認劑量是否合適（要用醫院電腦登入）

      

      

4. Right Time

   1. 常見縮寫：BD, TDS, QID, Mane, Nocte，給藥時間可參考下圖

      

      

5. Right Route

   1. 口服（PO）、靜脈注射（IV）、肌肉注射（IM）、皮下注射（Subcut, SC）

   2. 有些藥囑會寫PO/IV，這時候就要自己判斷哪種途徑最適合病人

      

6. Right Documentation

   1. 給藥後需立即在 medication chart 上完成紀錄

   2. 給藥後，在對應的時間簽名，格子很小所以會簽縮寫 (initial)

      例：Da-Ming Wang → DW

      

      備註：

      1. 注意OSCE考試題目給的日期，不要填錯日期

      2. 澳洲日期是 dd/mm/yyyy 日/月/年

   3. 若沒有給藥，依照下圖原因填入右側縮寫，縮寫要記得打圈

      

PRN 藥物

1. 給藥前需看一下上次給藥時間，必須超過4-6小時

2. 計算一下是否超過一天的最大劑量 Max PRN dose/ 24hrs : 30mg

3. opioid 類藥物為高風險藥物，需要雙重核對，兩位RN核對後一起到病人單位給，兩人都要簽名

   備註：止痛類藥物給完後，要記得再評估pain assessment, vital signs, sedation score

補充：高風險藥物 High Risk Medicines - AAPINCHEN

• 以下高風險藥物都需要兩人核對後才能給

A - Anti-infectives（抗生素）： Amphotericin, vancomycin, and aminoglycosides, but may also include others

Ap - Psychotropics（精神科藥物）：Clozapine, lithium and depot injections

P - Potassium and concentrated electrolytes Injectable electrolyte preparations （鉀離子或其他電解質製劑）：

potassium chloride and magnesium sulphate, but may also include other medicines

I - Insulin （胰島素）：All insulins

N - Narcotics and sedatives （麻醉藥與鎮靜劑）： All opioids, sedatives may include benzodiazepines and other sedating agents

C - Chemotherapy agents （化療藥物）：Cytotoxic chemotherapy

H - Heparin and other anticoagulants （肝素、其他抗凝血劑）：Heparins and all anticoagulants, including the New Oral Anticoagulants

E - Epidural and intrathecal agents （硬膜外及鞘內）：All medicines administered vis epidural or intrathecal route

Ne - Neuromuscular blocking agents （神經肌肉阻斷劑）： Atracurium, cisatracurium, mivacurium, pancuronium, rocuronium, suxamethonium, vecuronium

Once only/STAT medications

所有STAT, once only 藥物會寫在這個區塊，隨時注意醫生有沒有新增

Telephone orders 電話醫囑

電話醫囑在還在用紙本的機構還是很常見

1. 藥物確認：需拼出來

2. 劑量確認：15mg (o : one five) (x : fifteen)

3. 兩位RN核對 （一位先聽完寫下來，再由另一位RN聽一遍）

VTE prophylaxis 靜脈血栓預防藥物

住院病人因為活動減少、手術或疾病，血栓風險會增加，因此常需要進行預防。

非藥物的 VTE 預防措施 (Mechanical prophylaxis) :

1. TED stockings（抗血栓彈性襪）

2. SCD (Sequential compression device) (間歇性氣壓壓迫裝置)

Variable dose medicine 劑量會常調整的藥物

劑量會依照病人情況一直調整的藥物會寫在這一區

在台灣，護理紀錄多以 SOAP 的方式呈現，強調護理問題導向。然而，在澳洲臨床實務中，採用的是系統式紀錄。依照身體各大系統逐一條列評估與觀察結果，呈現病人在該班次的整體狀態。

這篇寫給護理學生以及準備或計畫進入澳洲臨床的你。

護理紀錄格式

1️⃣ 書寫工具

• 使用黑筆或藍筆(黑筆更合適）

2️⃣ 時間格式

• dd/mm/yyyy（日／月／年）

• 例：17/01/2026 14:30

3️⃣ 錯誤更正（Error）

• 不能塗改、不能立可白

• 正確做法：

  • 用一條線劃掉錯誤內容

  • 寫上 “Mistaken entry”

  • 簽名

例：
BP 120/80 Mistaken entry [Full name] RN

4️⃣ 句子寫完後的空白

• 句尾若有空白，必須：

  • 畫一條橫線填滿

  • 簽名

• 目的：避免事後被加字

5️⃣ 使用可接受的縮寫

• 只能用認可的醫學縮寫

• 不確定的縮寫 → 不要用

例：
✔ BP、HR、RR、IDC

6️⃣ 簽名格式

每一筆正式紀錄都要包含：

• 職稱（RN / EN / SN）

• 全名（工整可辨識）

• 簽名

備註：如果是紙本病歷，每句句尾都要劃一條線 ; 範例內容僅供參考，臨床上可以寫更詳細一些。

及時護理紀錄 Contemporaneous Nursing Note

及時護理紀錄用於記錄非例行性、突發或急性事件，例如病人症狀惡化、跌倒、疼痛突然加劇、呼吸困難等。

此類紀錄必須：

• 在事件發生後盡快完成

• 紀錄的時間必須是「事件實際發生的時間」

• 內容需客觀、清楚、不帶個人推測

一筆完整的及時護理紀錄通常包含：

1. 病人主訴（主觀資料）

2. 護理評估結果（客觀資料）

3. 已執行的護理處置

4. 病人對處置的反應

5. 後續計畫或是否升級處理

小小範例：
